Position: Case Manager

Giving Home Health Care provides in-home health services for those with illnesses contracted while working in the
nuclear energy programs of our country during the cold war and ongoing nuclear programs.

  • Location: Las Vegas, NV
  • Job responsibilities:
    • Directly works with the RN Case Manager, Administrator, and other management personnel.
    • Work on new patient admission packages.
    • Aid in the preparation of care plans.
    • Help prepare and process new patient assessments.
    • Work with patients and patients’ Doctors to process renewal and new service applications
    • Cross-train on billing processes.
    • Daily Patient Case Management documentation and record keeping.
    • Accompany patients to doctor appointments, advocate for them, and explain program requirements.
    • Regular review of cases with all team members, Nurses, CNAs and PCAs.
    • Perform home site visits to train and instructs team members.
  • Requirements:
    • Strong communication skills.
    • Confident and sure yet warm with your presentation.
    • Must have excellent reading, writing and computer skills.
    • Must be great at organization with ability to manage multiple projects.
    • Must have and follow a well-developed value system, is loyal, and interested in serving humanity, as you
      will be working with others that believe and live the same way.
    • Required minimum of an associate degree or equivalent.
    • 1-year case management or management experience.
    • Knowledge of Medical terminology or medical experience is a must.
