Position: Case Manager

Giving Home Health Care provides in-home health services for those with illnesses contracted while working in the
nuclear energy programs of our country during the cold war and ongoing nuclear programs.

We have an immediate need for a full-time case manager. If you enjoy advocating for those you take care of,
understand the dynamics of dealing with old age and chronic illness, and are ready to bring your resources, solutions, and

  • Location: Albuquerque, NM
  • Job responsibilities:
    • Help prepare and process patient assessments and plans of care
    • Work with patients and their providers to write and process medical paperwork for their home care
      renewals every 6 months and for new service codes
    • Work with providers to get the proper durable medical equipment (DME) approved for the patient
    • Cross train on billing processes
    • Daily patient case management documentation and record keeping
    • Travel to and attend patient Dr appointments with them and help them advocate for appropriate home care
      and DME
    • Travel to and perform patient home visits to strengthen your relationship with the patient and their family
      members
    • Provide training to patient, home health aides, and nurses as needed
    • Weekly check ins with all team members, nurses, and home health aides in each patient home you
      manage
  • Requirements:
    • Recent case management experience
    • Recent health care experience
    • Familiarity with medical terminology
    • Warm, confident, and persuasive communication skills
    • Excellent reading, writing, and computer skills
    • Ability to learn different technology programs quickly
    • Great organization skills and the ability to manage 25-30 patients on case load
    • Must have a strong value system, be loyal, and interested in serving humanity
    • Must have a driver’s license, car insurance, and reliable transportation for travel to patients’ homes and
      Dr’s appointments
    • Must undergo and pass a background check

  • Benefits: Begin after successfully completing a 90-day training/orientation period.
