Home Health Care Services

All care is prescribed by your physician and a reliable schedule is maintained by our Case Managers. Additionally, your family is not alone. Giving Home Health Care is able to train family members to provide a portion of your needed care and compensate them under the program.

Is Home Health Care Right for You?

Some of the services include:

  • Family Member Caregiver:
    • Many of your needs can be met by a family member. We are able to train and pay your loved ones to provide you with care as you deal with health complications.
  • Range of Care Levels:
    • We can provide various levels of care, ranging from one nursing visit per month up to 24-hour care depending on your care needs. Though the conditions our patients face are often chronic, our goal is to maintain or improve our patients’ quality of life by working with your doctor to establish a care plan appropriate to your needs.
  • Monitoring for Changes in Condition:
    • Our nurses are trained to perform thorough head-to-toe assessments at each visit. These frequent assessments allow for early detection of changes in health status—permitting for prompt treatment.
  • Perform Doctor-Ordered Interventions in the Home:
    • Our skilled caregivers will follow your unique care plan and perform interventions specific to your care needs. Our nurses can assist with medication management and administer breathing treatments per the doctor’s orders.
  • Nutritious Meal Preparation:
    • Our nurses and caregivers can assist with meal preparation according to the patient’s nutritional needs.
  • Identify Patient Needs:
    • Our skilled nursing staff will be able to identify durable medical equipment needs or required alterations in care plans.
  • Maintain a safe and hygienic living environment free of trip or fall hazards:
    • Your family needs rest too. Our caregivers can take on these duties during their time at your home and lift this burden from others.
  • GHHC may assist also with arranging your transportation needs.
